"Blue Skies, Green Grass"

Does anyone here know the origin of the phrase "Blue Skies, Green Grass"? I came across it in an article from a teaching journal: "A fantasy novel usually follows the "Blue Skies, Green Grass" theory." Does the author just mean that fantasy novels tend to be pastoral? In that case, why does she mention that fantasy novels tend to have cities?
