Police Clearance Certificate

Dear English speakers,

I have attempted to translate a police clearance certificate. It is very funny since it is supposed to be used abroad ONLY yet no English or other language versions are available. And I couldn't even find the English version of the names (let alone the texts) of the acts they refer to... Could you please check for me if there are any mistakes, anything that sounds odd...

The above certificate is intended to be used abroad exclusively.

Budapest, 21 December, 2009.

The current document is valid for 90 days from the date of issue.
The certificate may only be used with a documentary proof of identity.
The notice of appeal may be made to the issuing body within a period of ten days with the current document and evidence attached to it.
The certificate may exclusively be used to prove facts marked on the data transfer request form filled out by the applicant.

The transmitted information may contain specific data (criminal personal information) set in the Act No LXIII of 1992 on the protection of personal data and the publicity of data of common concern. For this reason the information obtained can only be handled and used for data collection requirements and procedures in accordance with the rules laid down by Act No XLVII of 2009 on the criminal record system, the register of judgments delivered by the national courts of member states of the European Union against Hungarian citizens, and the records of criminal and police biometric data. In case of data-security infringements and infringement of confidentiality (in particular, unauthorised or incorrect processing of data) the user is liable to prosecution.
