Russian translation

Hi everyone
A colleague and friend of mine is leaving work soon, and has given me a book to write a 'goodbye' message in for her. I wanted to include a sentence or two in Russian and wondered if the following was a relatively accurate translation of "Good luck! I wish you happiness and success. Don't forget about us!" I have a feeling something is wrong with it, but for the life of me can't see what.
